i have data in one pic. how can i use data validation like in another pic

i have excel data shown in below pic.

now how can i use data validation in such a way that i will have Consultation tye(checkup and emergency) and animal type(dog bird cat rabbit and hamster) and when i choose from consultation and animal i get total average that is in excel data
