I need help recreating this screen shot and adding functionalities to it using html/javascript.

When the user clicks on submit, display a reservation confirmation with the following information on the screen.

Name:

Member/Non Member (only 1 of these options)

Sport:

Date and time of reservation:

Rate: For members, display the rate as $35 per hour, for non-members display it as $50 an hour.

If the user does not check the I accept checkbox, show a dialog prompting the user to do so.
