I want a project in visual studio in C#. and a database sql file for this project.

This should be a heart disease prediction system database management project with following requirements.

1. welcome form which leads to login form. if user is not registered, move to registration form. login form further leads to main menu.

1. main menu form should contain button for "go to diagnostics".

2. diagnostics button should take us to another form " prediction system" in which it takes symptoms from user to diagnose the disease.

3. if the disease is diagnosed, a report is generated that contains all the information of user that was taken in registration form and tell the patient that you have been diagnosed with a heart disease. it should tell the user the required diet plan and medication.

4.if disease is not diagnosed tell the user "based on your symptoms our system is unable to predict". if you want to book an appointment with the doctor, you can book here.

5. In doctor's appointment form there should be choice of 2 doctors a heart specialist and a heart surgeon. once the patient selects the doctor it should ask the patient for the appointment date, if the date is already booked before ( validation for appointment), it should generate an error and ask for the date again. if a valid date is entered, it should display the "appointment booked" dialogue.

6. once the appointment is booked, it should ask the user for feedback that is stored in the database with all the other credential of user.
