The focus for the change assessment will vary depending on the type of project as follows for implementing EMRs

  1. TheType Aproject uses this assessment to gather valuable information for verifying the felt need for the change and for designing the change initiative based on understanding the organization's readiness and leadership capacity for change.
  2. TheType Bproject uses the assessment to answer questions retrospectively about change readiness, need, and leadership in the case of a specific change that has already been implemented. In addition to reviewing documents, this assessment provides a great opportunity to access the experience of people who were stakeholders in this change process.

Please respond to the following prompts:

  1. Design: Discuss your design for the assessment including:
    • The specific approach(es) you used to conduct the data collection and why you chose it/them. Please include any questionnaires, structured interviews, formats, etc. that were involved in this process.
    • The resources that were most helpful in your design process.
    • The people who were involved in the data collection.
    • How you analyzed the data.
    • The effectiveness of the approach used in gathering the data and any alterations you would have made to your process.

    • As a result of the assessment, will the outcomes of the project need to be altered in any way?OR Should the outcomes of the project have been altered to support better results?
  4. Challenges: What ambiguity and paradox do you see in your organization and how can those be reconciled?
  5. Reflection on Your Personal Role: Reflect on the process of change assessment that you are going through in your organization. (Definitely answer this one in a narrative. It might be two to three paragraphs or more.)

  6. Management Systems: This question is asking you to update your preliminary thinking in Change Project: Part 1.
