In System Analysis and Design, create a complete questionnaire form which includes closed and open-ended questions.

Provide an additional word document which indicates which parts of the problem description or final system the question is trying to provide more information for. For each question, list whether it classifies as an open-ended or closed question.

In the same word document, indicate how and to whom you will distribute your questionnaire and how and when you will follow up with the users.
