In this section, you describe your evaluation plan. Based on the lectures on surveys and data tools $($chapters $11$ and $12),$ what information are you planning to collect to measure the effectiveness of your health program? How often are you going to collect this data? How will you utilize the data that you collect? Review chapter $2$ on evaluations and utilize the key vocabulary to describe the type of evaluation you plan to conduct if your health service program is funded. What do you plan on doing with your evaluation when you have your results? Do not answer these questions in one sentence responses. Your plan should follow a narrative format. Organize your information so that your reader understands all of the steps you will take to evaluate your program.$]$
