The clinic is located near a new shopping mall in a busy section of the Damansara. Saujana Damai Health Clinic staff includes four doctors, three registered nurses, four physical therapists, and six office staff workers. The clinic currently has a patient base of 3,500 patients from 275 different employers, many of which have insurance coverage. Anita is the office manager of Saujana Damai Health Clinic. She supervises the staff, including Ahmad, Mimi, Farid, Lisa, and Ching. Ahmad handles office payroll, tax reporting, and profit distribution among the associates. Mimi is responsible for the maintenance of patient records. Farid handles most of the paperwork concerning insurance reporting and accounting. Lisa has the primary responsibility for the appointment book, and her duties include making reminder calls to patients and preparing daily appointment lists. Ching isconcerned primarily with ordering and organizing office and clinic supplies.

Each of the six office staff people has one or more primary responsibilities however, all members of the staff help out whenever necessary with patient records, insurance processing, and appointment processing. You are required to build an automated system for HappyPet Veterinary Surgeons called as Veterinary Practice System (VPS). Currently, the veterinary surgeons do everything manually and they want to automate their processes in order to provide better services to their customers. The purpose of the HappyPet VPS is to handle and record all the information of the owners, pets, treatment required, payment of bills, details of Insurance plans and appointments and to highlight cancelled and missed appointments. Assume that you have interviewed the owner of HappyPet Veterinary Surgeons and he provide you with the following scenario: -

HappyPet Veterinary Surgeons

The HappyPet Veterinary Surgeons' practice is located in Damansara Utama, Selangor. It employs four veterinary surgeons (vets). Being a city-based practice the clients are owners of domestic animals such as cats, dogs and caged birds. Some of the vets are specialist in particular types of animal (for example, one is an expert in treating reptiles kept as pets) while others do not have a particular specialism but can treat most types of animal.

A record is kept of the owners who have registered with the surgery and their pets. Details recorded include the owner's name, address and contact details; the pet's name, animal type (for example: dog, cat), pet's date of birth and date registered with the surgery. Some owners register their pet when they first need treatment; others register them when they acquire the pet, in advance of them ever needing to see a vet.

When an owner wants their pet to be seen by a vet, they bring them to the surgery. Appointments are made in advance. Details are recorded of the appointment date and time, the vet and the pet; if an owner is bringing two or more animals to the surgery each is treated as a separate appointment. After the appointment date recorded, it is whether the pet attended the appointment or some owners make an appointment but cancel it and do not turn up. The surgery wants to be able to identify owners who regularly make appointmentthat they do not keep.

Details of any treatments prescribed for pets are kept, including the date prescribed, which vet was involved, which appointment and any necessary notes. There are a number of standard treatments (for example, injection against cat influenza, clipping dog claws, de-worming and putting an animal down), each of which has a code and a description. A pet treatment may consist of something other than a standard treatment (for example, neutering). An appointment does not necessarily result in any treatments. Owners may also be provided with a standard treatment (such as worming tablets) without bringing in their animals for an appointment.

The surgery in association with an insurance company, offers an insurance plan for pet owners. The plan can be for one or more animals and covers all or part of the cost of treatments. Details are kept of the plans taken up by owners including the insurance plan number, the date it was taken out, which animal or animals it applied to and for which treatments a plan has been used. Not all owners have an insurance plan for their pets.
