Keep it simple: Don't worry about turning down/overlapping the vapor barrier, etc. Just use straight floor area (25'40') for the calculations.
You are NOT responsible for reinforcing, pour-break material, or formwork.
Materials:
6 mil. vapor barrier -100' by 10' roll - $65
2500 psi concrete - $105/CY (includes delivery)
Labor:
Laborer installing vapor barrier - Hourly rate $16hr, productivity rate 500SFhr
Laborer placing concrete - Hourly rate $20? hour, productivity rate 10CYhr
Laborer finishing concrete - Hourly rate $20? hour, productivity rate 250SFhr
