Instructions:

1. Construct the control charts in Excel, each problem takes ONE tab. Submit a SINGLE Excel file (with TWO tabs).
2. To minimize the possibility of typos during data transfer, please copy and paste the data table from this document to a new Excel worksheet.
3. Start interpreting a chart ONLY AFTER the chart has been found to be in statistical control (or stable). If the first iteration of the chart is not stable, remove those data points which are out of control and reconstruct the chart in the second iteration (to confirm it is now stable).
4. After constructing the charts, there are a few questions you need to provide brief answers to. These answers should be just a few sentences in length. You can type them in a blank spot right on the same Excel tab (by the charts).

Case 1:

Detroit Central Hospital is trying to improve its image by providing a positive experience for its patients and their relatives. Part of the "image" program involves providing tasty, inviting and healthy patient meals. A questionnaire accompanies each meal served, asking the patient, among other things, whether he or she is satisfied or unsatisfied with the meal. A 100-patient sample was drawn everyday over the past 31 days and yielded the data in the table below. Construct a control chart in Excel that plots the percentage of unsatisfied patients. Set the control limits to include 99.7% of the random variation in meal dissatisfaction rate.

Based on the information from the control charts you constructed, answer the following questions:

Is the process in statistical control? If not, what are the days that should be investigated? What are other possible recommendations you want to give to the management for continuous improvement on patient satisfaction (as demonstrated in the survey results)?
