There are ten sub-competencies of values and ethics that can be applied in healthcare and these include placing the interests of the patient first to promote health and health equity, respecting the dignity and privacy of patients, embracing diversity, valuing various cultural roles and responsibilities, support healthcare delivery and work collaboratively, developing trusting relationships with patients and their families, manage ethical dilemmas through population centered situations, acting with honesty and integrity, and finally, maintain competence in scope of practice (Slusser et al., 2018). A few of these sub-competencies have shown at my current practicum organization:
Respecting dignity and privacy- At a 24-hour residential care home, the patients are at the organization around the clock; the center is their home. Therefore, they take it very seriously to honor patients' privacy and dignity by setting ground rules for entering rooms, and schedules, and adhering to patient preferences such as what clothes to wear, what to eat, what to watch, and when and who to socialize with.
Support health care delivery through collaborative practice- Each member of this organization has set roles and responsibilities that must ensure are completed for the patient's care. This includes nursing working on authorizations, documentation, and medications, while operations work on billing, regulations, staffing, and training. Working collaboratively between departments increases collaborative communication and enhances the patient's experience.
Maintain competence- Continued medical education, credentials, and ongoing training is a requirements for employment at this organization. Operations as well as the Nursing team ensure all staff are trained efficiently based on CMS guidelines as well as receive continued education through monthly and annual training. Operations utilize audits to ensure all staff are up to date on continued education as well as offering additional training to specialize their skills.

The new director of nursing (DON) at my practicum site is an African. Though she has been in America for a while, she still has a strong African ascent, as we all do, but the workers are yet to get used to it. This has become an issue in the facility, as some of the nursing team members sometimes use her ascent as an excuse for neglecting their duties. They claim they did not understand her and could not carry out the instruction she gave. Instead of asking for clarification, they go about talking behind her back. This makes her job more complex, and the administrator had to address it promptly before the health outcome of residents was affected.
VE6 - Develop a trusting relationship with patients, families, and other team members (CIHC, 2010).
I was with the social worker in one of their meetings with a family member whose brother was a resident at my practicum site. The way this family member talked about his brother was so impressive that everyone commended him. He knew and understood his brother very well, and the social worker and the treatment team were pleased. But then there was another family member who knew nothing about her sister except for when her paycheck came in. It is even difficult to get her when she is needed. It is easier to build a relationship with the first family member than with the second one. The care team enjoys it when family members are involved in the care of their loved ones. They answer promptly when they are called about any issues and provide information when needed. This helps with better health outcomes.

Not being competent in one's profession can have a negative impact on health outcomes. Fortunately, that's not the case with my practicum site. For a while, my practicum site did not have a DON. So the Assistant director of nursing (ADON) did her job and that of the DON with the administrator. She was very competent, understood her role, and used her knowledge to appropriately assess and address the needs of the patients and populations they served. When the state came in, she was the one who was there to provide everything the state needed, and my preceptor praised her a lot, and I wondered why the facility was looking for DON when somebody already does the job very well. This is where certification comes in. Because she is not a certified nurse, she could not fully assume the role. However, her competence helped the facility when it was needed the most.
