It is important to keep in mind when to accommodate and when to modify.  Many teachers get these terms confused and use them incorrectly when speaking or planning,

Define accommodations and modifications.  What are they?  Who are they used for?

For ELA, Math, Science, and Social Studies I need 3 generic accommodations and 3 specific accommodations/modifications for a student with Autism in middle school.

In order to come up with specific modifications/accommodations, think about a particular lesson topic. No LESSON PLANS but have to list the general lesson topic.  For example, ELA Lesson topic:  Poetry. Science lesson topic:  Weather patterns

SO THIS IS WHAT IT WILL LOOK LIKE:

Math:

Lesson Topic:

-3 general accommodations/modifications

-3 specific accommodations/modifications

ELA:

Lesson Topic:

-3 general accommodations/modifications

-3 Specific accommodations/modifications

Science:

Lesson Topic:

-3 general accommodations/modifications

-3 Specific Accommodations/modifications

Social Studies:

Lesson Topic:

-3 General accommodations/modifications

-3 Specific accommodations/modifications
