Gender: The options are M $($Male$)$ or F $($Female$).$

School: The options are Chestnut Elementary, Hickory Middle, Poplar High.

Grade: The options depend on the school selected. For Chestnut Elementary, the grades are K$,1,2,3,4,5.$ For Hickory Middle, the grades are $6,7,8.$ For Poplar High, the grades are $9,10,11,12.$

Athletic Activities: This field is only visible for Middle and High School students. They can select up to two athletic activities. The options depend on the gender selected earlier in the form. For males, the options are Baseball, Football, Soccer, Basketball, Track, Cross Country. For females, the options are Softball, Volleyball, Soccer, Basketball, Track, Cross Country.

Optional Student Insurance: The options are Yes or No$.$

The form also calculates the activity fee based on the following rules:

Elementary students pay a flat $$50$ fee.

Middle School students pay $$100$ plus $$50$ for each athletic activity.

High School Students pay $$100$ plus $$100$ for each athletic activity.

Student Insurance adds $$300$ to the total.
