Make an ERD in 3rd normalized form please
The Mount SQL Hospital has been serving the ABC University community since 1957 and has recently
received a large donation from a group of the universitys alumni. The donation aims to help the hospital
rework its currently facilities and technologies. Your work over the past few months to create a new
database for the hospital to use has been very well received. A recent meeting with the Board of
Directors discussed expanding the database to include more information.
Mount SQL is divided into three primary departments: General Medicine, Surgery, and Psychiatry. Every
department has a head physician that oversees all other physicians in that department. Mount SQL
keeps record of each physicians name, position, SSN, and their department of expertise. Each physician
is trained in several procedures; however, there have been cases where a physician has performed a
procedure which they have not received training. Mount SQL has been known to perform a broad range
of procedures, from the simple Follicular Demiectomy to the experimental Complete Walletectomy.
Each procedure has a cost associated with it on file which is typically indicative of the complexity of the
procedure.
There are currently 10 Nurses working at the hospital. All nurses work under the hospitals head nurse,
Carla Espinosa. Nurse records contain their name, position, SSN, and whether they are registered in the
hospitals wallectomy project. Every floor block in the hospital has a Nurse on call who is tasked with
assisting and monitoring the blocks patients. In some scenarios, a Nurse can be put on call to several
different floor blocks. Nurses are given a start time and duration when they are put on call for a floor
block. Mount SQL has one facility with a single 4-floor building. Each floor has three blocks:
Block A Located on the East side of the building
Block B Located on the West side of the building
Block C Located in the center of the building
Every floor block contains multiple rooms; a room can be of type Single, Double, or Triple which
corresponds to the number of available beds in the room.
The hospital also keeps record of every patient seen. Patients must provide their name, sex, a valid
address, phone number, and insurance information. Patients that live in the area or frequent this
hospital also tend to see the same physician at the hospital. This doctor is their Primary Care Physician
and is documented alongside patient information if applicable. Every patient is encouraged to make an
appointment before being seen by a physician (With the exception of emergencies). Appointments
involve a nurse to prepare the patient, examination room number, patient, and a physician if applicable.
The hospital keeps record of each procedure the patient undergoes. This information includes the
physician performing the procedure, the patient undergoing the procedure, the date and time the
patient arrived, the assisting nurse (If applicable), and the original appointment number. A patients
original appointment could result in multiple procedures being performed therefore the hospital tracks
the start and end date for the length of their stay in a room.
A physician can prescribe medications to a patient. Every medication includes a name, brand, and
description of the product. A given prescription includes the physician prescribing the medication, the
patient receiving, and the medication. If the prescription was given as a result of an appointment, the
appointment number is also included.
The company has hired you and your team to propose a new relational database to house this
information. After youve designed and proposed your database, the company will supply their data and
you must import it into the new database. After the database has been created and filled with the data,
the company will likely have several questions to test the efficiency of the new system and to see what
kind of information can be pulled.
