The writer provided a notice of Medicare Non Coverage with a last cover date of June 26, 2025, with liability commencing on June 27, 2025. The writer also provided the notice of transfer/discharge indicating a discharge date for June 27,2025. Ms. Bassin asked for an extension but the writer kindly explained she has met all of her goals and has made great improvement since admission. Despite explanation Ms. Bassin expressed that due to the swelling in her legs and feet she would like to have more therapy. The writer will express her desire for more therapy to the interdisciplinary team. The writer explained that if the request for an extension is denied she has the right to appeal. Ms. Bassin was provided the number for Livanta and the appeal process was thouroly explained. The writer proceed to explain that if the appeal is denied, she also has the right to remain in the facility as a private pay patient. She was informed of the private rate of $650.41 per day for room and board and that therapy sessions vary form $80 to 80 per session. Ms. Bassin took notes of the information provided. She is also aware she has a pending bill for copay for the month of May and June. She will ask her son Lawrence to bring her check book to write a check and complete the payment prior to discharge
