Services Department. Management Services is the department responsible for management engineering activities at the hospital. The process of discharging or transferring patients is complex and involves several departments. For this reason it was decided to use a team approach to solving the problem. In selecting the team members, all departments that are involved in the process were invited to participate. To address this issue, the following problem statement was developed and presented to those invited to attend the meeting: Evaluate and improve the overall process of discharging or transferring patients from the time the order is written to the time the room is ready for a new patient to better meet the requirements of our customers. This statement is more an action statement or goal than a problem definition. This was done to assure that the process was viewed as a positive step to improve the service provided to the patient and not one of assigning blame. The end to meet customer expectations is also identified to keep the team focused on a common goal. This definition was not easily accepted because of the prejudices many team members brought to the meeting. Suggestions such as "make the environmental worker clean faster" or "reduce patient escort response times and we can all go home" were discussed at the meeting. Team Membership A cross-functional team was developed with members from all the departments involved in the process. Department heads were invited and encouraged to send someone directly involved with the process to represent their department. The final team membership included the following personnel: 1. Head Nurse, Med. Surgical Floor 2. Head Nurse, Telemetry Floor 3. Manager, Admitting 4. Manager, Environmental Services 5. Supervisor, Patient Escort 6. Manager, Management Services 7. Management Engineer The team consisted entirely of management personnel except for the management engineer. Some department managers were at times defensive and took every comment made by the members of the team personally. The fact that the team consisted of only management personnel was not a problem in any way. They were selected based on their expertise and ownership of the problem. A team leader was appointed, who worked hard to maintain the team's focus and guide it to its goals. The team leader was the Head Nurse, Med. Surgical Floor. Services Department.
