The report that was given by telephone nurse at the hospital included the following: Mary had an ischemic stroke 10 days ago. She has right-sided weakness in her arm and leg. Mary is left-handed. She has dysphagia, is currently NPO and has some garbled speech. She is a diabetic and is receiving Glucerna tube feedings at 75ml/hour per a PEG tube. She gets sliding scale insulin injections q 6 hours with Novolog insulin. She will receive Occupational, Physical and Speech therapies on the rehab floor at this nursing home. Mary has a history of type 2 diabetes mellitus, recent ischemic stroke with dysphagia, hypertension for 25 years and has been taking a statin for 15 years for high cholesterol and triglycerides. Mary was still teaching nurse aide classes at a local college until she had this stroke. Mary is an RN. Mary is hopeful she will regain her strength but is really upset about the NPO status and her swallowing issues. Recent vital signs: BP 140/82 RP 72 RR 14 SPO2 95% - Pain in back and coccyx currently that she rates 4/10 She does have a stage 1 pressure injury on her coccyx. Her serum albumin level is 2.8 Mary has been married for 49 years, has 5 adult children, and 19 grandchildren, two of who are great grandchildren, her husband reports. Her husband states he and Mary attend a Nazarene Church and that he placed Mary on the prayer chain at their church
