Survey Design

BACKGROUND

Your client owns a smoothie shop in Oshawa, Ontario. Lately, they have been considering

adding a drive$-$thru window in hopes of increasing sales. The construction costs are quite high,

so they have hired you to develop a short survey to determine if they should pursue the idea.

They are also interested in exploring potential ways to innovate the drive$-$thru approach to get

the most out of the potential investment and differentiate the brand experience.

INSTRUCTIONS

$1.$ Develop a short five$-$question survey that addresses the business problem, using the

survey design principles covered in weeks $7$ & $8.$

$2.$ The survey must include the following:

a$.$ One Likert scale

b$.$ One open$-$ended question

c$.$ One ordinal question

d$.$ One categorical question

e$.$ One question of any type you chose

$3.$ Submit the survey as a PDF to the Class Activity #$4$ assignment folder on DC Connect

by the end of class today.

$$ The evaluation criteria for this assignment includes marks for professionalism, which

includes spelling, grammar, clarity and general presentation of the assignment,

consistent with minimum business standards.

$$ Late assignments will be subject to a $20\%$ per day late penalty $($including weekends$),$ up
