The dentist office needs to store information about patients, how many times the patient visits the office, the work (procedures) that must be performed, procedures performed during visits, charges and payments for treatment, and laboratory supplies and services. Assume there is only one dentist, so there is no need to store information about the dentist in the database. However, there are many patients. Patients make many visits, and the database should store information about the procedures performed and the services provided during each visit, and the charges for each of the services. There is a standard list of service charges, kept outside the database. The office uses three dental laboratories that provide supplies and services, such as fabricating dentures. Each patient is assigned a unique ID. Patient's ID, name, address and phone number are to be stored Each procedure has a unique code and fixed charges A patient may have more than one phone number which is/are to be recorded The date of each patient's visit to the office is to be recorded Each dental lab has a different name and a different address A patient never visits the office more than once on the same day For each payment, amount, date, time, and the patient for whom the payment is made are recorded
