For this assignment, you will use your teams research question from Team Assignment 1 to assist you with individually creating a survey questionnaire. Although you will develop this assignment on your own, you will collaborate next week to combine the individual surveys into one comprehensive survey.

• Introduction - Use your teams research question to guide the survey. Provide an introduction to describe your teams research topic along with context learned in your literature search.
• Create a Survey Questionnaire:

• Based on your refined research question and using the readings to guide you, develop a set of questions that will help you collect the information you will need to answer your research question.
• The survey should be composed of a variety of question types, including but not limited to, Multiple-Choice, Single Response Scale, Multiple-Choice, Multiple-Response Scale, Likert Scale Summated Rating, and Numerical Scale.
• Use the Westcliff University Survey Question Guidelines below to create your survey questions.
• Under each question, include a one sentence statement USING ITALICS which explains why you recommend each question be added to this survey.
