Observe each area noted in the document and complete the form. a. Explain to the individual what you are assessing and why it is important to the individual. 3. Make recommendations for modification of the individual's work station for any item(s) checked on the OSHA Workstation Checklist. b. Verbalize to the individual why each modification is necessary c. Document observations and recommendations on OSHA Workstation Checklist 4. In addition, ask the individual to demonstrate one activity that he/she may perform consistently during the day other than using the computer work station. Observe his/her performance and provide feedback regarding proper body mechanics. Record your observations and your recommendations for change below, if necessary. 5. Have the Concorde Career College employee fill out the evaluation form that provides feedback about your performance. 6. Turn in the completed OSHA Workstation Checklist as well as the feedback form with this sheet to your instructor
