Question: On 11/05/20 a Production employee was using an Amp Machine to Crimp Terminals onto wires. While performing the operation one of the Terminals got stuck

On 11/05/20 a Production employee was using an Amp Machine to Crimp Terminals onto wires. While performing the operation one of the Terminals got stuck in the Anvil (crimping) portion of the machine. The employee then opened the door in an attempt to remove the stuck part, while removing the terminal the employee stepped on the foot pedal which then actuated the machine causing the anvil (crimping portion) to come down on the left index finger of the employee. The employee was then taken to the hospital where it was found that the employee suffered a fractured finger with lacerations.
