Objective:
The objective of this Part is to review 3 sample team performance evaluation forms
provided. Offer a critique for each one. Based on your critique select one sample form
most appropriate for assessing your group members. If none of the forms suits your
group you are welcome to design one of your own. You will be required to justify your
decisions in a brief report.
Part Specifics Form and Critique:
Your group will be required review sample evaluation forms and to select a form that will
be used to assess your group members in Module 14. Specifically you will need to:
1. Review the 3 Sample forms offered on FOL.
2. Using best practices from the Course Readings, offer a critique of each form.
Your critique should include:
o 3 positive features of each sample form. You must include supporting
data from the Course Readings for each feature listed.
o 3 recommendations for improvement for each sample form. You must
include supporting data from the Course Readings for each
recommendation for improvement listed.
3. Select one sample form most appropriate for assessing your group members.
Provide a reason for your selection. If none of the forms suits your group you are
welcome to design one of your own. This form will be used in Part 2 when you
assess each group member.
4. Use the Group Work Appraisal Part 1 Templates in FOL for your report.

5. All 3 Evaluation Form Samples can be found under the Group Appraisal
Assignment section in the Content in FOL.

1. Completed Template:
This Part will be submitted by a designated group leader to the group Submissions
Folder on Friday@11:59pm in Module 5
2. Individual Work:
Post your work contribution to assignment in the group Submissions Folder on or
before Friday@11:59pm in Module 5. If you do not submit evidence of your
contribution, you will receive a grade of 0.. There is a My Contribution
Template available in FOL.
Important:
Part 1 must be submitted in order to be eligible to submit Part 2.
Due to the unique nature of this assignment, late submissions will be
accepted in order to continue with the assignment. A deduction of 20% of
Fanshawes Academic Integrity Policy applies to this assignment. Cite all of your sources
the assignment per day will be made for late assignments. After five days,
assignments will no longer be accepted. This penalty equals 1% off of your
final grade per day.
