Patient and process outcomes are interventions for practitioners, interventions for patients, change in knowledge, change in practice and change in economic outcomes. (Medves, 2010). The idea of disseminating evidence to make improvements in health care is choose strategies that will have a successful outcome. Today health care has evolved and the way of obtaining information have changed as well. Today we serve a generation who is has more contact with both computer and those who prefer to collect information the traditional way via literature. I goal is to capture all audiences (Elderly and young). The strategies I will choose is website, patient portal and monthly magazine. Thedissemination element could be written on paper (i.e., pamphlets, flyers, booklets), verbal (i.e., using tele- phone), or written or verbal using ICT (i.e., e-health, m- health, websites, multimedia, telemedicine, patient re- minder, etc.). The dissemination could be done individually, in groups, or massively. (Chapman, 2020). Consideration should be towards all audiences, giving all patients options on how to receive care.
