These social determinants cause a problem with accessing healthcare, stable housing, and nutritious food. Major Challenges Limited availability of primary care, especially specialists, makes it difficult to receive timely medical care. This causes patients in my community to use the emergency department as their primary care or their only care. Proposed Changes One change would be to increase access to healthcare with telehealth, especially for those with limited mobility. \"Telehealth has been used to promote healthy behaviors and condition management, and there have been promising effects observed, including increasing patient participation and satisfaction and reducing rates of chronic illnesses\" (Butzner & Cuffee, 2021). Implementing mobile food pantries can improve access to a variety of nutritious foods. Food insecurity is associated with malnutrition and obesity and can potentially contribute to chronic conditions, such as cardiovascular disease and diabetes (Villa et al., 2022). Speak with local transportation providers to help those with limited mobility. This can help improve access to individuals who had a lack of transportation. Risk Assessment Instruments A good risk assessment tool for assessing my community is the Health-Related Social Needs (HRSN). This is a tool that can help identify patients' needs that can be addressed through community services.
