Problem (10 points): A medical office wishes to optimize its scheduling procedure and hires you to design a DB following these specifications. Every patient has a unique patient ID, name (first, last), address (street, city, zip, state), an email, a list (possibly empty) of phone numbers. Every patient has a list of next of kin (name, relation, contact phone) with at least one person in this list. Every physician has a unique employee ID, unique medical license number, name (first, last), work email, work phone number. When a patient contacts this medical office for the first time, the patient is assigned a physician who will be the leading physician for this patient. Every office physician can be the leading physician for many patients, however, there are physicians who are not assigned as a leading physician to any patients. When a patient schedules an appointment, the appointment is typically scheduled with the first available physician. For every appointment scheduled by a patient, the office stores the information regarding the physician who conducts this appointment (not necessarily the patient's leading physician) and the exam room where the appointment takes place. Over time a patient can schedule multiple appointments. Every physician who works in the office is assigned to appointments (many appointments over time). Every exam room is defined by its unique room number, and the list of equipment items available in this room. Every generated appointment has an appointment ID, date, time. Also, for every appointment a physician leaves comments about the appointment. Every patient has at least one credit card associated with this patient (unique card #, expiration date). This is a family practice, so several patients can share a credit card. If a patient has several credit cards associated with him/her, then one of the cards is as the primary card
