please help me construct a time sheet i do not understand this

Experiential learning opportunities are an essential way to focus on the knowledge, skills, and abilities you need to realize your career goals. You have specifically selected this course as an opportunity to earn practical experience in your chosen field to help realize these goals. In your timesheet, record your experiences and time spent on a weekly basis during your experience. (Note: It is strongly suggested that you spend at least 10 hours per week in the experiential learning activities. Number of hours must be documented and final certificate of completion will be required as part of your final project.) Your weekly entries should address the following topics: 1. Indicate which days of the week you completed any activities in your experience. 2. Indicate the title of the activity. Examples include video titles, website url's, where and what you engaged with the tool/technology etc. 3. Indicate what type of activity it was - was it a video, a book, a web page, a tutorial, a lab, a discussion, tutoring session etc. 4. Note the number of hours spent on each activity. Format A word template has been provided. You may submit your time sheet as an excel document if you prefer. Either format is acceptable.
