Please help me formulate this because I am stuck on making cohesive sentences.

CONTEXT:

- I am conducting an APA research survey on pet preference: Fish or Birds

- My 2 variables being measured are: Lifestyle (sedentary or active) and Pet Preference

INSTRUCTIONS:

1. Materials
   • Describe your survey, including:
     • What it measured, the number of questions, at least one open-ended sample question and at least two closed-ended sample questions, including the rating scale used (e.g., 5-point Likert scale) and the response options.

I used 1 open ended and 2 closed-ended all of which are "enter the blank" but how do i formulate that into the "Materials" section in APA?

Please provide examples...
