I would like to know if you think that external influencing factors that impacted the results in the VAM New York example could be classified as random effect? By definition, random effect (based on the book) is something that explains deviation from the expectation. I viewed the external influences as factors (hunger, sickness, and stress etc.) that caused deviation from what was expected had they not been present. I'm interested to hear your thoughts as well as others
