  2. Based on the scenario outlined below, create a risk assessment matrix for a fictional SAIT hospital and a 3-4 page risk assessment report for the senior leadership of that hospital. Write your report based on that target audience.

Scenario

You have been provided with the following information about the facility to help prepare the risk assessment:

The hospital is located in Calgary, Alberta, Canada. Information on the community's disaster risks are available on the City of Calgary website https://www.calgary.ca/csps/cema/explore-calgarys-disaster-risks.html.

The SAIT hospital is in a flood-prone area close to a river. A decade ago, the hospital basement flooded. More recently - two years ago - flooding came close to breaching sandbags set up around the perimeter.

The hospital is located on the outskirts of the city in an area that has occasionally had grassfires during periods of drought. The closest treed areas are approximately 2 kilometres from the hospital.

  1. Use the risk matrix framework discussed in Unit 3 to populate the risk matrix template on the next page with the following hazards:
  • Flooding
  • Severe Winter Weather (snow/ice storm or blizzard)
  • Wildfire
  • Tornado
  • Earthquake
  • Pandemic
  • Loss of utilities (gas, electricity, water)
  • Cyber attack
  • Terrorist attack/active shooter incident
  1. The matrix sets out a scale of 1-5 for both likelihood and consequence. In the table, define each level using key words. You may add footnotes for further explanation.
  2. The result of the matrix sets out a risk score ranging from 1-25. There are four colour code categories. Define each category using key words. You may add footnotes for further explanation.
  3. Once you have plotted each hazard on the risk matrix, create a risk assessment report.
  4. The risk assessment report should include the following sections:
  • Introduction. Explain the purpose of the report and summarize its contents.
  • Risk assessment findings.
  • Summarize the findings of your risk matrix by ranking the hazards from 1-9 (ties/equal rankings are OK) and explain the rationale for each ranking (point form or table format is acceptable). Include your risk matrix either as an embedded table or appendix
  • For the top five hazards that present the greatest risk to the hospital, list three specific risk management strategies for each hazard that can be used to prevent or mitigate the risk. Recommended risk management strategies should be supported by cited research.
  • Recommendations and Next Steps
    • Summarize the top priorities. The hospital's senior leadership have advised that they have limited financial resources for risk mitigation and have asked you to present a three-year plan that sets out the risk management strategies that should be addressed immediately and those that can be implemented over a longer time period.
  • Conclusion: Briefly wrap up the report.
