It is vital that you capture the reasoning behind your actions. Include specific details about what you are thinking about the situation, how you came the to the decisions you did, why you are taking the actions you are, why they are wise, or justified etc.

3. Finally, think about what outcomes you expect to result from your action plan in both the short and long term?
