Make a Survey Form using HTML form components that take the following inputs: 1. Name 2. Phone Number 3. Email Address 4. Date of Birth (simple text input, we will change this later) 5. Gender to select options from Male, Female or N/A 6. Check-in boxes with the following options: Project 1, Project 2, Project 3 7. Multi-line text box for Comments 8. A submit button on the right You can design the HTML file but it must have the aforementioned 7 input fields.You can change the text or the options as you wish. However, make sure you have tooltips and placeholders for each input. Using styles are not mandatory but make them internal (no external css link).
