Technologies: HTML, CSS (you can use bootstrap if you wish or any other CSS framework), JavaScript (no frameworks or libraries allowed), MySQL, Apache, and PHP (no frameworks or libraries unless specified).

Email sender and PDF reports generator (both optional, in both cases you can use libraries)
