Proposal should include

1. provide an introduction to the change effort being addressed in your action plan, and give a name to your change effort.

2. Describe the community where the action plan will occur. include basic characteristics- physical, political, economic, demographics, and history.

3. clearly identify the unmet needs in the community and identify the unmet needs that your action plan will target. why did you choose this need in particular?

4. identify assets, resources, or capacities (abilities) present in the community. also, identify what bases of power in the community your action plan will use and how will you use these in your change efforts.

5. Discuss how you will develop resources needed for the action, including people/support and building your budget

6. Explain what approach your efforts will take and explain why the strategy or tactic was considered for your desired outcome.

7. Describe what key concerns your efforts may encounter in order to maintain itself internally. What challenges might you face as a change agent organizing this?

8. Clearly explain how your organization will evaluate the success or failure in reaching the desired outcome.
