Study the scenario and complete the questions that follow: As the Training Manager in a large gold-mining company, you have been called in by the director of Human Resources, Pat Steele, to attend a meeting on training issues. Pat has sent you a report on the number of accidents and injuries sustained by mine workers in the last months. There were five serious injuries and ten minor incidents. Safety procedures were ignored in almost all these cases. Safety equipment is not checked before use, and the correct use of the equipment is not adhered to. The previous week, an injured worker was given first aid treatment only two hours after being hurt. The first aid kit was not replenished after it had previously been used; nobody reordered stock for it. Pat Steele is not impressed with the attitude of the workers towards safety at the mine.
