Scenario 1
Mrs. A (32) was brought to a homeless shelter by the police. She was found sitting on a bench at the Eaton Centre crying. Her 3 young children (ages 8, 6, and 4) were sitting with her. When you approach her to learn about why she is at the shelter she says, "I was kicked out of my house by my husband after we had a fight about money. First I took the children to my parents' house. Dad said that we could not stay there with them because I belong with my husband. I don't want to return to my husband. He has been physically attacking me and saying that I am no good for many years. Now I am afraid that he will hurt the children. We are here with nothing except the clothes that we are wearing." When you ask her about self-harm, she states, "I have never thought about this. Who would look after the children?" She then adds, "I know how it feels to have someone close die by suicide; my sister killed herself 2 years ago."
Scenario 2
Mr. B (26) has been attending the drop-in centre where you work for several years. One day he approaches you and states "I have been homeless for 5 years. This was okay until last week, but now I need help. Last week, my only friend Stanley died after taking cocaine. I was with him then. I was also using cocaine. It was just awful. I tried to help him but he died anyway. Why did he have to die? Since Stanley died I have felt so guilty. Was there something that I could have done to save him? Now I am afraid to go to sleep because I have nightmares about Stanley. I feel so bad that I want to die too. I will jump in front of a subway train."
Scenario 3
Mrs. C (33) is a successful journalist who has had two admissions to the hospital for major depression. Her most recent admission followed a serious suicide attempt (overdose of her antidepressant). Now ten months after discharge, Mrs. C. is brought by her friend to the clinic where you are working. When you ask Mrs. C. why she is there she tells you, "This afternoon my husband, Bill, told me that he does not want to be married to me anymore." When you inquire further Mrs. C. admits, "I am so sad and angry at him for doing this to me." She continues to say, "At least I know that if I find this to be too much I have a large bottle of sleeping pills that I can take to end it all."
