Situational Awareness loss warning signs/red flags are ambiguity, confusion, fixation, not communicating, not address discrepancies, violating minimums, failure to set/meet targets. After reading and watching the video analyze the situation awareness loss of flight 401. Describe what deficiencies you noticed and what red flags stood out that all of the fight deck had loss situational awareness.
