Patient A:
Patient A is a 72-year-old female with an admitting diagnosis of failure to thrive. She can make some small position changes in bed, but requires a Hoyer lift to get out of bed. She often needs to be "boosted" up in bed when she slides down and she is frequently incontinent of urine. Patient A recently had a PEG (feeding) tube placed due to difficulty with swallowing and she receives tube feedings overnight to meet her nutritional needs.
Patient B:
Patient C is a 63-year-old male who is being treated for throat cancer. He is independently ambulatory and does not require any assistance with ADLs. He has been receiving radiation therapy for the past four weeks, which makes him tired and nauseous. He has been spending more time in bed than usual lately and doesn't have the energy for anything other than getting to and from his daily appointments. Patient C has lost 10 pounds since his diagnosis a few months ago. In addition to the nausea, he finds it very painful to swallow and cannot find many foods that he can tolerate.
