The biggest challenge to achieving public consensus on this issue is stigma. HIV remains one of the most misunderstood and morally judged conditions in the world. Add in the politics of the borderimmigration debates, national security concerns, and drug war rhetoricand you've got an issue that many people would rather ignore than confront head-on. Until we address the cultural and political barriers surrounding HIV, we can't expect to make meaningful progress.
