Root Cause analysis is a tool that was used during an investigation to why someone may have been hurt when working on a ladder in a room by themselves. Root Cause analysis worked wonders within this scenario.The team and I used the 5 whys to understand the cause of the accident. This situation could have been easily avoided so that why we used a simpler approach to finding out the cause of this accident. The root cause analysis method encouraged a great team to joint their minds together to understand what went wrong within this scenario. The issue within the accident was a systematic issue that involved a worker working within a room by himself. This is also not mentioning that the employee who had gotten hurt was 70+ years old. This incident was not a complex investigation rather a simple one that needs verification on how the employee was hurt. Root cause analysis created the overall idea of lone working and working at height could have been the cause. This helped the accident investigation team develop well rounded corrective action to ensure the problem would not resurface. Fault tree analysis could have worked for this situation but due to the lack of technical expertise and the time consumption of this method it was a lot easier to use root cause to understand the accident better.
