08 Scroll Read Mode 2. The color of chlorine gas, used as a disinfectant in the water treatment process, is greenish- yellow, and it has a higher density than air. It is highly toxic and is a hazard to the human who handles it. On June 23, 2017, one of the operators, who did not sleep well before coming to work, made an error, and the toxic chlorine gas was released into the atmosphere, causing breathing problems to two nearby working operators. Emergency medical services (EMS) were called upon to deal with the situation. Management of the company appointed three human reliability experts to analyze the situation and identify the HEP. Experts analyzed the system and identified three possible human errors; an operator can fail to close the chlorine gas valve when required, an operator may ignore the concentration alarm, and the operator's attention may be diverted to some other tasks. While conversing with an operator, experts found that he lacked experience working in a similar job and was in a rush to complete the task. Experts also learned that the operator did not have any formal training to perform the job he was assigned to do. Experts agreed that training could have the most significant effect on the success probability of the chlorination process. Extreme hot weather also affected his ability to perform his job. Considering two tasks in the past, experts identified failure HEP of task 1 as 0.01 and task 2 as 0.06. They also determined that the SLI values of 70 and 30 can be used for tasks 1 and 2, respectively.
