Experts also learned that the operator did not have any formal training to perform the job he was assigned to do. Experts agreed that training could have the most significant effect on the success probability of the chlorination process. Extreme hot weather also affected his ability to perform his job. Considering two tasks in the past, experts identified failure HEP of task 1 as 0.01 and task 2 as 0.06. They also determined that the SLI values of 70 and 30 can be used for tasks 1 and 2, respectively. Using the SLIM technique, calculate HEP for all human errors and identify which human error has the highest chances to occur? Provide at least four recommendations to improve the system performance? (Hint - there are five PSFs!) 3. A reliability analyst and human factor expert want to assess the probability of human error to close a critical valve, as per operations need. An operator is believed to have familiarity with the system and knows which valve needs to be closed but gives little attention to the task and does not respond correctly. Scheduling department identified operator will have only four hours gap between his two eight hours shift - affecting his sleeping habits. The operator is doing the job of a chemical engineer while his credentials are in electrical engineering. It is also observed that one operator can complete the task, but an additional operator is available to assist him.
