Each patient has the flexibility to book multiple appointments with one or more doctors at the clinic, allowing them to seek the care they need. Doctors at our clinic can see many patients throughout the day, but each appointment is specifically scheduled between one doctor and one patient. In the case of emergencies, where immediate attention is required, patients don$$t need to book an appointment in advance. However, to keep track of such visits, these emergencies are recorded in our appointment book as $$unscheduled$.$

When a patient keeps an appointment, it leads to a visit with the doctor they booked. During this visit, the doctor will diagnose the patient$$s condition and provide any necessary treatment. After each visit, we make sure to update the patient$$s medical records to maintain an accurate medical history, which is crucial for ongoing care.

Each visit to the clinic results in a bill, which is issued by the doctor who attended to the patient. Our billing process allows flexibility, as patients can pay their bills in multiple instalments if needed. Additionally, a single payment can cover more than one bill, making it easier for patients to manage their healthcare expenses.

Patients have the option to pay their bills directly, or they can choose to have the bill sent to their insurance company for payment. If an insurance company covers the bill, the patient is still responsible for paying any deductible, ensuring that all costs are appropriately managed.

b$.$all entities, relationships $($including names$)$and attributes that are relevant,

c$.$primary keys identified, and

d$.$include relationship participation $-$participation $($optional $/$mandatory$)$symbols and cardinality.

The E$-$R diagram should be completed using the standards of this unit. Use Microsoft Visio to create the E$-$R diagram.

The group should also complete the Student Contribution table for this stage of the Assignment and submit it along with your E$-$R diagram.
