SOAP Note: When asked who sent you here, he correctly replied, "The judge." When asked how come you're here, he responded, "Talking about charge or to see if I'm competent to stand trial?" Counselor provided information. When asked a charge is the thing people say you did that is against the law. What is your charge, he stated, "Burglary of a dwelling." Counselor confirmed. When asked what does the charge mean, he said, "A felon." Counselor provided education. When asked what did the police say you did, he answered, "Broke in someone house." When asked on what date did this happen, he replied, "I'm not sure." When asked what about what time, he responded, "I think it was in the afternoon.
