SOAP Note: When asked who sent you here, he replied, "The court." Counselor told him the judge. When asked how come you're here, he responded, "To see if I'm competent or not." Counselor elaborated. When asked a charge is the thing people say you did that is against the law. What is your charge? Mr. Mitchell expressed that he did not feel comfortable discussing his charges. When asked what does the charge mean, he replied, "I don't know." Counselor informed. When asked what did the police say you did, he said, "I don't know." When asked on what date did this happen? About what time? He responded, "Don't remember the date." When asked what is a crime, he commented, "A person did something." Counselor provided a definition of a crime.
