Sperry & Sperry (2020) indicate a valuable case conceptualization provides an appropriate explanation for presenting concerns and anticipates barriers to treatment in order to facilitate the treatment plan effectively and competently. When looking at John's case it is important to consider the four components of a case conceptualization: diagnostic, clinical, cultural, and treatment from a biopsychosocial perspective.

John is presenting with signs of depression, low self-worth and feeling stuck. After determining the question of "what" we can then explain John's presenting factors through a biopsychosocial lens to determine the "why". John may have a genetic disposition to depression, a medical concern or hormonal imbalance influencing the biological factors. The psychological factors may include his avoidant coping skills, emotional neglect, patterns of disrespect by family/partner and feelings of unworthiness. Social factors include his strenuous familial relationships (parents, siblings, spouse, in-laws, and children) and poor interactions with co-workers. It is also important to analyse the cultural influences such as his sexual orientation and identification as a Canadian male determine John's perspective of "what role does culture play". Evaluation and analysis of these presenting factors will help in determining an effective treatment plan with John to promote healing. It is also important to discuss the factors that might present a challenge for John to implement the treatment plan, to foresee these concerns and work with John to develop skills to ensure success (Sperry & Sperry, 2020).
