Using a Memorandum of Understanding format (MOU) of your choosing, please make a document, that adequately covers the agreed-upon guidelines and interactions, between you and the principal, for your organizational interviews, assessment, change recommendations, and final meeting with the principal to review and receive comments on your analysis, change recommendations, and recommended change plan.

This document must minimally include estimated or actual meeting dates; purpose of agreement; scope of agreement; access to another staff person (for the second interview); confidentiality of assessment and recommendations between you, the principal, and me; and signature/date lines for both you and the principal. Remember there's no change for your services, and don't feel obligated to use all the "legalese" seen in the examples. Keep it simple.
