The following seven examples present case management services. Identify the method described and a minimum of two roles that will be necessary. ________________________________________ 1. Creswell Family Services provides case management to families, especially those with children. Families must apply for services at the Creswell office. Intake information is gathered so eligibility can be determined. Services available include educational classes for mothers, childcare, food and clothing donations, and one-night temporary housing.

2. Waverly Emergency Shelter houses young women who have run away from home. They are placed in the shelter from two weeks to 30 days. During that time the young women are housed and fed and given educational and psychological assistance. Many clients "run" after they have been placed at Waverly. They resent the supervision and want to be on their own.

3. Intensive Case Management Program serves persistently mentally ill clients, people who have needs that can't be met in two or three months. Case managers "do what it takes" to help their clients. The goal is for clients to live in the community. They teach clients daily living skills and provide transportation, mental health care, recreation, and other services.

4. At Edison High School, case managers assist the school with identifying their resources and helping them link with community resources. There is a student health and human service action team, where the school and the community come together. The case manager makes sure that each student receives the services needed. For example, if a teen is pregnant, that teen receives post-pregnancy planning and health and mental health care. Ultimately, the parents and the teens themselves assume the case management responsibility.

5. SURI Youth Services provides services to youth on the streets. Some kids receive shelter. Other youth want to remain independent, so they receive food, clothing, or both. It is up to clients to choose what services they will receive.

6. At the Rodriguez Family Counseling Center, the case managers are gatekeepers for services. Most of their clients come from a hospital discharge planner. They interview clients over the phone to determine eligibility. The services they furnish are short term.

7. Helen Crowe Adjustment Training Center provides one-stop shopping for its clients. It has a residential program, a day program, and an independent living program. Clients enter needing residential care; the goal for most is independent living.
