The HIT Project is the implementation of an IT solution for a healthcare problem youve identified.

You identify the problem and who is affected by the problem (the target population)

Definition of a Target Population: A group that shares similar characteristics and is identified as your intended audience

Examples: patients with low health literacy, nurses who are at risk for alarm fatigue, people who miss their appointments at a primary care practice, etc.

Format: PowerPoint presentation, between 6 10 slides (not counting Title or Reference slides).

Required Content:

Introduction Slide (Title, group members)

Problem Identification (your needs assessment)

Why is it a problem? What is the cost of the problem (money, increased

hospitalizations, inappropriate use of the ER, mental or emotional distress, etc.)

Identification of Target Population

Who is the population? What are the challenges faced by this population?

Proposed Solution

What is your solution? How did you determine that was the best solution for your

population? What factors did you consider when you were deciding on the best option

for your population?

Proposed Implementation

How are you going to implement your solution?

What is the conceptual model or framework that you are following and explain how it is

guiding your implementation? Lewins, PDSA, Technology Acceptance Model, Health

Belief Model, Chronic Care Model, etc.

Measurement

How are you going to measure success? How often or when will you measure?
