This is an example of a Responsibility Report. Responsibility Reports may look differently than the one reproduced. Different computer programs will produce different types of sheets. They basically contain the same type of information. The heading will give the name of the organization and the department.

There should be a responsibility report for each cost center for each month. They are condensed into the Report of Operations. The Responsibility Report is a snapshot of timeit shows where a department is at that time in the fiscal year. The budget is a planning tool, but as you can see when you study the report, not all plans work out they way they are projected.
