$4)$ Each consultation has the following attributes: consultation$\_$date, Patient$\_$name,

Doctor$\_$name, medical$\_$note, as well as any number of prescribed medications and$/$ or

medical tests.

$5)$ Each patient may have zero or multiple consultations.

$6)$ However, each medical test $($e$.$g$.,$ blood test, X$-$Ray$)$ is owned by one and only one

patient. The medical test has its name $($e$.$g$.,$Lower Back X$-$Ray$),$ and the results

report as attributes. Make sure that The patient may register with more than one address.

$7)$ Each patient has associated with him$/$her zero to any number of consultations.
