Unit III Assignment Performing a Job Hazard Analysis (JHA) Identify a job that you routinely perform at work. If you are not currently working, think of a job you have performed in the past or use a job that you perform at home. Use the Sample Job Hazard Analysis Form to conduct a JHA of the job you identified. This form can be found in Appendix 3 of the Job Hazard Analysis document at https://www.osha.gov/Publications/osha3071.pdf Your chosen job must be broken up into at least five steps, with a JHA form completed for each step. Each form must include a Task Description, Hazard Description, and Hazard Controls. In the Rationale or Comment section for each step, apply the risk assessment matrix provided in the article by Lyon and Popov (indicated below) to determine the risk level for each identified hazard both before and after you apply your controls.
